    After enrolling in PSI please submit your Confirmation Letter, FHSU Waiver Form & Accept/Denial form, and explanation of insurance coverage by the deadline to the International Student Services Office in the Forsyth Library 036, lower level. When UnitedHealthcare emails you the Insurance ID Card, you must download this ID Card PDF, and submit it to the International Student Services Office ([email protected]) as well to complete your waiver process.
