  PLEASE NOTE: If you are enrolling in the University of Toledo beginning in the Fall semester, you are required to purchase health insurance coverage for the ENTIRE academic year even if you will be leaving after the Fall semester is completed. You also must purchase a PSI plan with a $0 deductible.

  After purchasing your insurance with PSI, download and print your insurance documents and then complete the university's online waiver through Aetna Student Health's website (linked above and within your PSI online account).
- The University of Toledo's Health Insurance Verification Form
  
  In addition to waiving the university's student health insurance plan online, you may also need to send the Health Insurance Verification Form via fax to 860-907-4730. If you receive this form and do not have access to a fax machine, you should complete the student section of the form and then send it to PSI at robert.honkisz@psiservice.com as an email attachment so that we can fax it for you.

  Scholars' health insurance must meet government-mandated requirements. PSI Student Plans I, II, & III meet these requirements.
