PSI answers benefit questions in WRITING. PSI does NOT answer
benefit questions over the phone. You are more than welcome to
call PSI for help with procedure and status questions. You may
also check our website for information.
Complete
the application form and pay the premium online with credit
card. Then retrieve the following insurance documents from the
PSI secured website:
1) Insurance ID
Card
2) Insurance Claim
Form
3) Policy
Brochure
4) Sales
Receipt
5) Insurance Confirmation
Letter
6) School Waiver Form (for student
insurance enrollees, if your school has such a form and PSI
has the form in file)
PSI does not mail
insurance documents.
You may login to your
account to renew your coverage, purchase new coverage for
yourself, your family, or your visitors, and you may also
change your information online, such as address, school name,
major, degree, etc.
After
buying PSI insurance online, you will be able to print the
following documents from the PSI secured website to register
at your school.

3. Policy
Brochure
4. Sales
Receipt
5. Insurance Confirmation
Letter
6. School Waiver Form (if your
school has such a form and PSI has the form in
file)
Some schools require international students
to purchase the school insurance plan, and they should grant
waivers if the alternative plan you purchase meets the
school's published requirements. In that case, you can present
the PSI Policy Brochure to your school. If your school has a
waiver form for alternative plans, please fax: 1-888-789-4499 (toll free)
or email the waiver form web link (if available ) or form file
to PSI at psi@psiservice.com. PSI will complete the waiver
form and put it in your account. You may then print it out and
submit it to your school. Please check PSI's website before
sending in the form because in most cases PSI has your school
form online.
In the event of injury or illness, the
Insured should:
If you are insured under the PSI
Medical Emergency Insurance, or you are insured under a PSI
Student Plan and your condition is an emergency, skip steps 1
and 2, and go to the nearest hospital.
1. Visit the
campus Student Health Center
2. If the Student Health
Service is not available or further treatment is required,
contact Coalition America for a referral to a network doctor
(please check "How to find a network doctor" for instructions):
3. Present your insurance ID card and PSI Claim Form
to the doctor and follow the doctor's instructions.
4.
Complete a Claim Form with all itemized bills, statements, and
receipts attached, and mail to the Claims Division.

It
takes 20 to 25 business days for the Claims Division to
respond to your claims. If you do not hear from the Claims
Division after 30 business days, call the Claim Division to
check the status of your claim(s). If you can not get a
satisfactory answer from the Claims Division, contact PSI at psi@psiservice.com or 1-888-789-4488 (toll free).
Insurance in the US is
not welfare, it is business. The insurance industry is highly
regulated in the US and every state has an Insurance
Commission to protect the consumers' interests. But as a
consumer, you also need to follow the proper procedure and
take precaution to protect your own interest:
1) Read
the Policy Brochure and know your benefits;
2) Follow
the "Claim Procedure" when seeing doctors and making claims;
3) When communicating with the insurance company,
always write down the name and position of the person you
speak with, the phone number, and the date and time. Whenever
possible, ask the insurance company/hospital to communicate
with you in writing;
4) Keep a copy of everything you
send and receive from the insurance company.
These
steps will help you to receive all the benefits to which you
are entitled.

To find network doctors or hospitals in your area:
1) Click "Find Network Doctor", below, to find a Coalition America network doctor.
3) Read Disclaimer, then click "I ACCEPT BEGIN SEARCH".
4) Put 3722 in the Client PIN field search, click "Begin Search"
5) Put 35 in the "Search Mileage" field and fill other necessary fields.
6) Click "Begin Search" to find network providers. If you fill the zip code and State ONLY, there will be more results.

(In case of emergency or if there are no network doctors or hospitals in your area, you can see any doctor. The benefits will be the same as a network doctor. In such cases, attach an explanation when filing claims)
* Call
the doctor's office to make an appointment. You should
say, "My medical insurance coverage is through PPO networks on
my ID card: All Student Health Centers and; PPO Next; Intergroup; PHS." If necessary, say, "Underwritten
by American International Group(AIG)"
* Always
bring a Claim Form, which you can download from your PSI
account.
* If you are asked to pay, send
original receipts and a Claim Form to the address found on the
Claim Form, in order to receive your refund.

f) On the CONFIRMATION PAGE, you should state your reason(s) for the cancellation and attach copies of your support document(s). The refund is only effective once we receive the document/proof. The attached document(s) should be any one of the following:
i) proof of another insurance coverage with the same or overlapping coverage period
ii) airline ticket showing the enrollee will leave the USA
iii) letter from your university giving detailed reasons why your
PSI plan does not meet the school's requirements 'Unless we receive proof documents we cannot process your refund request'
g) After you have completed these steps, please click whether Proof sent by an email attachment OR by fax (use the icon from our site, also must have a tick here to be accepted)

NOTE: There will be no refund if the enrollee has filed claim(s), even if the claim(s) was/were determined NOT eligible for coverage.Claims will be denied if the enrollee applied for a refund.
2. Method to calculate the refund amount:
Refund = Original premium - Premium for used coverage* - Fee**
* Coverage is rounded to the nearest month for student plan and 14 days for visitor plan.
**Fee: $25 processing fee plus 2.5% of premium credit card charge.
Example: Original 12-month coverage is from 8/1/2008 to 8/1/2009 for $449, written refund request is received by PSI on 11/10/2008. Premium for used coverage will be rounded to 4 months (8/1/2008 to 8/1/2009). Premium for 4 months coverage is $164 ($123 for 3 month plus $41 for 1 month).
